Office Financial Policy

1. Dental Insurance – While you are always the one responsible for payment for dental services, we will accept assignment of your dental insurance as full or partial payment toward your account balance after your annual deductible has been met. We will estimate to the best of our ability your out-of-pocket share and this portion is due and payable at the time of service.

2. On estimates of large treatment cases, half the balance is due when treatment is started. The balance is due before the treatment is finished. This applies to crowns, bridges and dentures only.

3. A personal check will be accepted only if we have a valid driver’s license and social security number on file. If you do not feel we need this information, then you may choose to use your credit card or cash. No exceptions.

Managed Care Insurance: Payment is due the same day services are rendered.
